Welcome to The Karlfeldt Center. We’re honored to be part of your health journey. Here’s everything you need to know before your first visit so you can arrive prepared and make the most of your time with your doctor.
How to Schedule Your First Appointment
Scheduling at The Karlfeldt Center is simple. Call our front desk at 208-338-8902 during office hours or use our online scheduling system. Our team will help you select the right doctor and appointment type based on your primary health concerns. Office hours are Monday 9 AM–4 PM, Tuesday 9 AM–5 PM, Wednesday 9 AM–6 PM, Thursday 9 AM–5 PM, and Friday 9 AM–4 PM.
Completing Your Intake Forms (Cerbo Patient Portal)
The Karlfeldt Center uses Cerbo, a secure electronic health records system, for all patient intake and records. After scheduling, our staff will set up your patient portal account and send you a link to complete your intake forms electronically. Please complete forms at least 48 hours before your first appointment so your doctor can review your health history in advance. Forms include a comprehensive health history questionnaire, current medications and supplements, dietary and lifestyle information, and your primary health concerns. If you need help accessing the portal, call our office and staff will assist you.
What to Bring to Your First Visit
Please bring any recent lab work or medical records, a complete list of current medications and supplements with dosages, your insurance card if applicable, a photo ID, and any questions you’d like to discuss with your doctor.
What to Expect at Your First Appointment
Your first visit is designed to be thorough and unhurried — plan for 60–90 minutes. Your naturopathic doctor will review your submitted health history, conduct a comprehensive consultation covering symptoms, lifestyle, diet, and stress levels, and perform any indicated examination or on-site testing. Depending on your situation, your doctor may order advanced functional testing such as Hair Mineral Analysis, comprehensive hormone panels, stool analysis, or Autonomic Response Testing to identify root causes. You will leave with a personalized care plan outlining your treatment priorities, recommended therapies, supplements, and follow-up schedule.
Insurance and Payment
The Karlfeldt Center is a cash-pay practice. We do not bill insurance directly, but can provide a detailed superbill for out-of-network reimbursement with PPO plans. HSA and FSA cards are accepted. Please call our office for current fee information.
